Why PMs fail.

Because many PMs were never designed from failure modes.

Instead, they were copied from:

  • Old PMs
  • Generic templates
  • Historical habits

Good PMs answer three questions:

  1. What can fail?
  2. How does it fail?
  3. What maintenance activity prevents or detects it?

Without that logic, PMs become paperwork.

And paperwork doesn’t improve reliability.
